    Passport & Personal Information Form

    Please note that it is VERY important that any of the passport related fields are completed EXACTLY as they appear on your passport. If you do not, and your trip is ticketed with the wrong name, gender, or DOB information, you may incur additional expenses for correcting the ticket prior departure. The airline will not let you board if any of this information is incorrect.
    If you do not yet have a passport, please enter the information how you will on the passport application; which usually requires that it match your ID.


    Important message about deposit:
    In order to place your package on hold, we have to place a deposit on the package that is equal to the total price of your flight and $50 towards your hotel stay. While that price cannot be finalized until you are actually booking (because flight pricing varies), I can tell you that depositing this trip will likely be approximately $500 per person.

    Travel Insurance Information
    There are two travel insurance plans available, but the premium depends on which room category and the cost of your booking. The cheaper plan is $89 per person and is called VPP and provides a limited air and land voucher should you have to cancel your trip. The air and land voucher both likely have to be used within one year of the booking date for your trip (not the travel date). VPP+ insurance is $179 per person and will provide a full refund to the original form of payment if you have to cancel. For both plans, cancel for any reason is allowed up to 24 hours prior to departure, and then anything under that time frame has to be cancelled for a covered reason (for example personal illness/injury, loss of job. death in the family), these coverage claims usually do require proof when the claim is filed.
